How to Create an Activity Form and Attach it to an Activity

Build a custom form and attach it to an Activity so Account Holders and Instructors can fill it out in their portals

Activity Forms are custom forms that you attach to an Activity. When someone posts that Activity, they fill out the form, and their answers are saved to the Enrollee's timeline. Activity Forms use the same field elements as the Enroll Form builder, so if you've built an Enroll Form, this will feel familiar.

Create an Activity Form

Step 1 — Create the Form

Click the person icon at the top right of the screen and go to My Company > Forms > Activity Forms. Then:

  • Click the plus button to add a new form

  • Enter a title for the form

  • Click Save

Step 2 — Add Fields

  • Click the plus button to open the list of field elements

  • Enter a label for your question

  • Select the field type that fits the question

  • Click Add

Step 3 — Customize Field Settings

Field settings open automatically after you add a field. To open them later, click the cog icon next to the field. From there, you can:

  • Turn on Long Label Format if your question is longer than 100 characters

  • Make the question required

  • Change the maximum number of characters allowed

  • Choose an icon under Design by typing a keyword into the search bar, or keep the default icon

  • Adjust the width for Desktop/Tablet and Mobile by sliding the bar to a value from 1 to 12 columns. If some of your questions are long, open the form on a phone to check that it's easy to read.

  • Add Options for Single Select and Multiple Select questions, either one at a time or all at once by clicking Upload and choosing a CSV file

Step 4 — Add Follow-Up Questions With Conditional Logic

Conditional logic shows a follow-up question only when someone gives a certain answer. It works with Yes/No, Single Select, and Multiple Select questions. For example, if someone answers Not Good to How is the student doing?, you can have an Explain field appear.

  • Add the follow-up question as a new field

  • Click the return arrow on the follow-up question to nest it under the main question

  • Open the follow-up question's field settings and, under Visibility, choose the answer that should make it appear

  • Click Save

Step 5 — Organize Your Fields

Each field has a set of icons you can use to organize the form:

  • Up and down arrows — move a field above or below another field

  • Plus icon — duplicate a field

  • Cog icon — open field settings

  • Trash can — delete a field, then click Undo if you change your mind

How to Attach a Photo to an Activity Form

Do you need to allow an Account Holder or Instructor to attach an image or photo to an Activity? The best way to do that is to attach it to an Activity Form. You can learn how to create an Activity Form here.

Step 1 - Open Activity Form

Go to Activity Forms in My Company settings, and click the Activity Form where you want to attach the photo.

In this example, we're going to use a Progress Report Form.

Step 2 - Add Question Field

Once the form is open, scroll down to the blue plus button to add a new field.

Add your photo field. You can ask a question like, "Would you like to add a photo?" or shorten it to "Add a photo?" In that case, you would put that question as the label in a Yes/No field. Click Add, then Save.

Next, click on the plus down arrow icon (see below) to add an optional field based on the answer to this question, like such:

Step 3 -Add File Upload Field

Add a File Upload field called "Add Image" or "Add Photo."

When the field editor opens, under "Visibility," check the box to only show this field if "Yes" is the value. Click Save, and you're done.

NOTE: For this form to appear within the Activity, it will need to be attached to that Activity. To attach this form to an Activity, check out this support article.

How to Add the Activity Form to an Activity

Enrollsy allows you to add an Activity Form to a particular Activity so that Customers can access it on the Customer App. If you haven't yet created an Activity Form, learn how to do that here.

After creating and customizing the Activity Form, follow these steps.

Step 1 - Activities Tab

Go to the Activities tab (My Company > Activities). Click on the cog beside the Activity to which you want to add an Activity Form or create a new Activity.

Step 2 - Add Activity Form

Scroll down to Form Settings and under Attach Form, find your Activity Form from the dropdown list.

Click Save, and you're done!

Example

We added an Activity Form with an "Upload Form" field to the Intake Notes Activity. This allows Admin users to upload a Document to that Activity after it is posted to a timeline.

You can create any number of Activity Forms and attach them to specific Activities. Here are more examples of possible Activity Forms:
