Activity Forms are forms you attach to an Activity. Account Holders fill them out on the Enrollee's timeline in the Customer Portal, and Instructors fill them out in the Instructor Portal. You build Activity Forms in the same editor as the Enroll Form, using the same field elements.

NOTE: Account Holders need to choose a Class when they fill out an Activity Form within an Activity.
Use Conditional Logic for Follow-Up Questions
Many of the examples below use conditional logic. It shows a follow-up question only when someone gives a certain answer. You can add follow-up questions under Yes/No, Single Select, and Multiple Select fields.
For example, if How is the student doing? is answered with Not Good, an Explain field appears. To learn more, see [Conditional Logic](existing link).
Document Upload Forms — Collect Files From Account Holders
File Upload
Use this form when Account Holders need to upload a document to their account, such as an immunization report, vaccine card, emergency medical card, or intake notes. To create it, add a File Upload field.
Immunization Records
Some schools need immunization records, which usually come straight from the doctor's office. Create a simple form with one File Upload field labeled for the immunization report.
Food Allergy Form
Share a blank allergy form for Account Holders to download, fill out, and upload again. This form uses:
A Formatted Content field with instructions and a link to the blank form
A File Upload field for the completed form
A second Formatted Content field that reminds Account Holders to upload the completed form
Health Policy
Share a health policy and ask a screening question in one form. This example uses:
A Header with the policy name
A Yes/No question asking whether the Enrollee has any symptoms
A File Upload field with the policy document attached so Account Holders can download it
Enrollee Information Forms — Keep Details Up to Date
Emergency Contacts
Account Holders, Instructors, and Admin users can post emergency contacts to an Enrollee's timeline. This example uses:
A Header for the section
Short Text fields for the contact's name and relation
A Phone field for the contact's phone number
A Yes/No question asking whether to add another contact, which shows a second set of contact fields when answered Yes
Size Chart
Collect clothing sizes for things like summer camp attire. This example uses:
A Header and a Formatted Content field with instructions
A Short Text field for the Enrollee's name
Single Select fields for shirt, pants, shoe, sock, underwear, and coat sizes
Conditional logic that shows the right size options based on an earlier answer
Permissions
Get permission for field trips or extracurricular activities. Add a Yes/No question for each activity, such as a rock wall, water activities, inflatables, or the swimming pool.
Attendance and Daily Care Forms — Track the Day
Pick-Ups
Instructors and Account Holders can record who picked up an Enrollee. This example uses:
A Single Select field for the pick-up method
A Single Select field for who picked up the Enrollee, with an Explain field when Other is selected
A Short Text field for the name of the person who picked up
Make-Ups
Find out why an Enrollee missed class. This example uses a Single Select field with Sick, Holiday, and Other as options. Choosing Other shows a Short Text field so the Account Holder can say more.

Meal Count
Schools in child nutrition programs often track meal counts for each classroom and meal. This example lets Instructors record:
The Class name in a Short Text field
The date in a Date field
Counts for breakfast, AM snack, lunch, and PM snack in Number fields
Infant Feeding
Help Instructors track infant feeding times. This example uses:
A Single Select field for the type of food
A quantity question that changes based on the food selected
A Short Text field to describe strained or table food
A Single Select field for the next feeding time
Diaper Change
Record diaper changes on the Enrollee's timeline. This example uses:
A Multiple Select field for the reason for the diaper change
A Long Text field for notes
Incident Report
With an Incident Report form, Instructors can report an incident right after it happens, and the report is available in the Customer Portal right away. You can base it on your current incident report or a state form, or [download an Incident Report template](existing link). This example uses:
A Header with reporting instructions
Short Text fields for the facility name, county, child care consultant, injured child's name, and time of incident
A Number field for the facility ID
A Date field for the date of incident
Single Select fields for the type of injury, body part injured, and medical treatment received
Classroom and Progress Forms — Share Updates on Learning
Progress Report
Send student progress reports through an Activity, such as one named Student Progress Report. This example uses:
A Header for the report
A File Upload field for the progress report document
A Single Select question asking how the student is doing, with an Explain field when Not Good is selected
A Yes/No question asking whether to add a photo, with a File Upload field when Yes is selected
Homework
Post homework assignments with a due date. This example uses:
A Long Text field for the assignment details
A Date field for the due date
Announcement Forms — Share News and Events
Most announcement forms use the Formatted Content field. With it, you can add a block of headings, paragraphs, bulleted or numbered lists, links, images, video, or tables. Select the ellipsis on the field to open the formatting menu. To learn more, see How to Customize the Enroll Form.

Share the details Account Holders need for a school event. This example uses:
A Formatted Content field for the event heading
A Date field for the event date
Short Text fields for the time, location, and who posted it
A Long Text field for event details
Birthday Announcement
Celebrate an Enrollee's birthday. This example uses:
A Formatted Content field with a Happy Birthday graphic
A Short Text field for the Enrollee's name
A Date field for the birthdate
A Long Text field for comments

Post a message from the director. This example uses:
A Short Text field for the subject
A Long Text field for the message
A Short Text field for who signed it
Staff and Enrollment Forms — Handle Absences and Withdrawals
Withdrawal Form
Give Account Holders an easy way to give notice when an Enrollee leaves your program. This example uses:
A Header for the notice
A Short Text field for the names of the children withdrawing
A Date field for the effective date
A Yes/No question asking whether a two-week notice is being given
A Short Text field for the primary parent's name
A Date field for the current date
Staff Call Out
Staff can use this form when they need to call out. This example uses:
Short Text fields for the employee's name and time of call
A Date field for the date of absence
A Multiple Select field for the reasons for absence, with follow-up questions for Other and Illness
A Short Text field for how long the employee will be out
